1. Acceptance of these terms

By creating an account or using deaform41 (the “Service”), you agree to these Terms of Service. If you do not agree, do not use the Service.

2. What the Service is

deaform41 is a record-keeping tool for DEA Form 41 — Registrant Record of Controlled Substances Destroyed. It helps you author, finalize, retain, and produce Form 41 records.

The Service does not provide electronic signatures and is not a signature provider. Witnesses sign on paper. We capture structured data, generate the official PDF, and provide tamper-evidence for finalized records.

3. Your responsibilities

You are solely responsible for the accuracy and legal correctness of the information you enter, and for your own compliance with the Controlled Substances Act and DEA regulations. deaform41 does not provide legal advice.

You are responsible for maintaining the confidentiality of your account credentials and for all activity that occurs under your account.

4. Acceptable use

You may use the Service only for lawful purposes and in accordance with these terms. You may not misuse the Service, attempt to access it in an unauthorized manner, or interfere with its normal operation.

5. Availability and changes

We may modify, suspend, or discontinue any part of the Service at any time. We may also update these terms from time to time; continued use of the Service after an update constitutes acceptance of the revised terms.

6. Disclaimer and limitation of liability

The Service is provided “as is” without warranties of any kind, whether express or implied. To the fullest extent permitted by law, deaform41 is not liable for any indirect, incidental, or consequential damages arising from your use of the Service.
